About Synera Health Group
Synera Health Group was born from lived experience at the intersection of medicine and managed care.
Its founder, Dr. Rafael Hernandez, is board certified in Geriatric Medicine and has spent decades leading large,
multi-market healthcare organizations through the transition from fee-for-service to value-based care. In those
roles, he was not only responsible for clinical outcomes, but for navigating full-risk models, payer contracts,
quality programs, and the operational realities that determine whether a practice thrives or struggles.
What Dr. Hernandez saw, repeatedly, was a widening gap between physicians and the managed care ecosystem
that governs their future.
Physicians were being asked to assume financial risk, meet increasingly complex quality benchmarks, and
operate like enterprise organizations—yet they were given little guidance, limited infrastructure, and almost no
strategic partnership. Many were excellent clinicians, but were forced to navigate capitation, RAF, HEDIS, Stars,
utilization management, and payer negotiations on their own. The result was burnout, underperformance, and
lost revenue—despite delivering high-quality care.
Synera was created to change that.
The vision was clear: build a physician-centric organization that understands both sides of the table—the
clinical realities of practice and the business mechanics of managed care. An organization that could guide
providers through complexity, protect their autonomy, and equip them with the tools, contracts, and
operational support necessary to succeed in value-based models.
From inception, Synera assembled a leadership team drawn directly from the managed care world. Executives
who have built networks, negotiated health plan contracts, launched markets, and operated under full-risk
arrangements. Professionals who speak the language of payers and understand their incentives, timelines, and
regulatory pressures. Individuals who have sat inside health plans and large provider organizations and know
how decisions are made.
Together, the Synera team brings more than 100 years of combined experience across multiple markets
